Buffalo Bill was partly based on Ed Gein, the cross-dressing serial killer who also inspired the character of Norman Bates. Like Gein, both characters are not trans, but identify and live as men, while their cross-dressing is private and ritualistic.

Jame Gumb was completely insane so identifying as trans carried the same weight as identifying as Napoleon, r23
In that universe, Hannibal Lector is a genius psychiatrist who is never wrong. If he said Jame Gumb isn't trans then that's that.

He doesn't meet the modern day criteria of being trans, r29. Anyone who wears a human skin suit is fucking nuts and loses the right to identify as anything. He was also rejected from a bunch of trans medical programs because the doctors knew he was crazy. It's in the book, movie or both.

R39 They disguised themselves as women to hide from the mafia, so it’s not a trans film; it’s a movie about how far men will go out of desperation. The humor is in how uncomfortable they are cross-dressing, not that they are into it. “Tootsie” and “Mrs. Doubtfire” are in the same vein.
