Definitely the spacing between your eyes - you can’t physically move them to be further apart, obviously. If you get a really good nose job you can give the illusion of wider set eyes, but that’s really more if your eyes are more average set - super close set nothing will help.
And to a certain extent you cant change the shape of your eyes either. Tori Spelling could never get rid of those big eyes.

One thing I’ve noticed is nose jobs where the lower part of the nose and tip have been whittled down, but the higher part of the bridge (between the eyes) is still thick and large. Britney Spears’ nose is an example. If surgeons can shave down Kylie Jenner’s lower face, seems possible to whittle down the upper part of the nose bridge.

My partner’s nephew was born with cleft palate. It really is amazing what plastic surgery can do. He has another surgery left, after about 6-7 through the years, but the plastic surgeon and orthodontist are miracle workers. You’ll still be able to tell he was born with a cleft palate, but the reconstruction is amazing.

R44 Believe it or not, the surgeons have a couple of different approaches to solving that problem. Generally the entire hairline is lowered, often in conjunction with a brow lift, though men may benefit more from hair transplants. Lowering the hairline with natural and masculine results is a bit trickier.
The lack of a Philtrum, generally cannot be fixed, nor can a tiny face on a rather large head (i.e. Charlie Kirk).

I saw a documentary years ago about a French surgeon who developed a way to correct really bad congenital facial deformities on children. I don’t remember the name of the syndrome but the kids are born with really widely spaced eyes and flat, nearly non-existent noses. They showed the surgery on screen - he basically peeled the face down so he could cut out each orbit and rejigger the structure of the face “like a jigsaw puzzle.” He used parts of the boy’s big toe to create a nose — it was one of the most amazing things I’ve ever seen.

R90 mouth width can be changed by a variety of procedures, some less invasive than others. I know a woman who had very tiny lips and got permanent makeup (tattoos) to make them look wider and change/even out her lipline, and then got lip filler, and it totally changed her mouth size. Kylie Jenner has had similar work.

Too much space between the nose and upper lip, which give a person an unattractive or apelike appearance. Stephen King here is an extreme example, but even if everything else is good it's one of those things that can keep a person from being beautiful.
Plastic surgery can't fix it, and nose jobs can make the problem a LOT worse - removing nasal tissue or putting the tip of the nose further up can make the nose-mouth gap larger or apparently larger. About all that can be done is lip fillers in the upper lip, which so often looks horribly fake that that it can also make things worse.
