Yes it can be simple self-hatred, internalized homophobia, Munchausen's, or all of the above. Christine Jorgensen was the first person to have a sex change and he was the quintessential self-hating gay man. It's too bad he never made it as an entertainer. He would have been a fabulous drag queen.

Yes and no, OP. A lot of the trans-identified today do very little actual transitioning, mostly low-level hormones and "top surgery". So, women get their tits chopped off and take enough T to get some facial fuzz, but they keep their female reproductive anatomy. Men may get breast implants but will keep their dicks. In many cases, they won't go through any actual physical transitioning at all, just dress creepily. The old school, full "sex change" is a thing of the past. The truth is most trans today don't want to change that much about their bodies, they just want to be special.

There has always been a sliding scale of gender dysphoria. At lower levels just "cross dressing" or maybe hormones and other feminization (or masculine) treatments are enough. For others like Bruce Jenner it's a bit more, then you have those for who only the full chop will do.

Problem for trans in the past is nearly all western and eastern societies define sex as gender; that is what lies between a person's legs. To be too much of one and not enough of the other meant living a life legally as a "man" or "woman. Pre-op or non-op trans thing was hard to pull off because no government or anyone else would issue identification documents that stated someone was "female" when they had a dick swinging between their legs.

It began to become clear even to places that pioneered gender reassignment surgery that medicine's answer to transgender just wasn't working. That is to get trans to have the chop, feminize them to extent possible then let them loose in world as "women". In large measure those post op trans were equally unhappy as they were pre-op. Worse they found themselves still facing same issues as pre-op. World largely still considered a post op trans a "man", if that is what he/she still looked like (see K. Jenner without makeup...). Straight men didn't want much if anything to do with a post-op trans either. And guys who were into trans largely want a chick with a dick.

Trans in numbers stopped playing the game. That is going along with how "miserable' their lives were with a dick in order to get sex change operations. Yes, you still see even young trans hopping up on that table. But good majority today are non-op, and don't have any plans on having the chop.

Ironically it is the six-foot-two line backers who look like Rosie Grier in a dress that turn out to be the most successful (as in content with outcome) post op trans who have the chop. Knowing from start they never would pass as female, but just wanted desperately to violently be rid of their dick.

What is the point of going through all the pain and trouble when you can just do what I did.

Trans right now has become a joke. Worse it is amazing how these individuals (some of who one has serious doubts about mental stability) have been allowed to suck all the air out of gay movement.

Gays have nothing to do with trans, never had nor never will. We kept them out of our bars and clubs (aside from entertainment) because gay men like other *MEN*.

It's getting so bad now you've got to set up elaborate screens for searching smut on your favorite site of choice otherwise these random MTF or FTM videos keep popping up.

Christine Jorgensen was also for the most part accepted as a woman. Many people were sympathetic to this idea that one can be born in the wrong body, and surgery can fix that. Perhaps if that was stressed, sex changes would be viewed like they are in Iran, as a treatment to a medical issue.

Instead they all aligned with the LGB which was worse for all.
