There are no more separate residencies. It's one unified system now. Prior, there had been dual accreditation programs since the 80s, and twenty years ago the AOA gave full endorsement to DOs doing MD residencies.

R23/VOTN: If you don't mind my asking, I am genuinely curious: Did you consider and/or apply to any allopathic colleges of medicine or did you decide during undergrad that you wanted to pursue osteopathic medicine and thus only applied to D.O. programs? If you did not consider any allopathic/M.D. schools, is there a specific reason, or set of reasons, why not?

Personally, I have a cousin who is a new PGY-1 "baby" D.O. who was originally planning on a neurosurgery residency followed by a neurotrauma-neurocritical care fellowship (as you know, that's 7 years of postgrad education on top of 8 years for undergrad + med school...ugh! No thanks!), but during his med school year 3 & 4 clinical rotations/clerkships & electives, he fell in love with the E.D. and emergency medicine. As such, following graduation, he applied to, and matched with, a standard 3 year emergency medicine residency. It's obviously very early in his postgrad residency education, but so far he has no regret(s) for choosing EM over neuro. He decided during undergrad, for whatever reason, he wanted to pursue osteopathy and to my knowledge, did not even apply to any M.D. schools. He had the grades, educational record & accomplishments, extracurriculars, recommendations (his Mom is a board certified family N.P., like me, so they know many M.D.s, D.O.s, A.P.R.N.s, P.A.-C.s, R.N.s, etc., who gladly provided letters of recommendation), etc., I believe he would have had zero problem getting accepted into an allopathic medical school--just as he did not have any problem getting accepted into his first choice of osteopathic medical school--if that had been what he wanted. However, it was not; he wanted to be, and now is, a Doctor of Osteopathy. We aren't very close, so I have never asked him why, exactly, he chose the D.O. route instead of M.D., so I figured if you don't mind answering, I would get your view(s) on the subject. Thanks in advance if you answer me!

I applied for both. I was a non-traditional student, and DO schools tend to be a little more accommodating than allopathic. I was 35 when I started, and I wasn't the oldest person in my class. I think I ended up where i was supposed to. It's hard to quantify this, but I interviewed, really liked the place, and had an acceptance letter two weeks later.

Would I have gone to an allopathic school given the chance? I mean, maybe? I'm a decent enough believer in vibes that I would have had to have clicked with the place. If I'd been that set on the letters, I would have gone to the Caribbean. And that wasn't going to happen.

As far as your nephew going for one over the other? I dunno, but I did have plenty of classmates who loved the manipulation part and the philosophy. DO is not the same as MD.

The admission criteria are much easier to meet for osteopathic programs compared to American MD programs. The professors at medical schools are MDs and PhDs who are experts in their fields. Professors at DO schools are other DOs and perhaps PhDs who work in small labs that are not very active. No MD would choose to work at a DO hospital or school. Third and fourth year medical students rotate at prestigious hospitals with top notch doctors. Third and fourth year DO students rotate at low-tier hospitals where you would not want to receive care. These years set the foundation and standard for lifelong learning that doctors must continue to do. The world leaders in any given field are MDs. This is no accident. It is extremely unlikely that DOs would ever advance their respective fields.

Residency training is not equal. Again, MDs train at the best hospitals in the country. DO residency programs are completely separate and are sponsored by basic community hospitals. MD programs can accept DO students, but this is by far the exception. There are very few DO residents in MD programs.

For confirmation, do a google search of “internal medicine residency programs” or “general surgery residency programs” in your city and compare these hospitals to “osteopathic internal medicine residency programs” or surgery programs in your city. The difference will be obvious.
