Patty Duke was a good looking woman for so much of her life
She was a beautiful child and young woman and middle-aged woman but in older age you'd never have known it - she's barely recognizable, dreadful dykey short haircuts and spectacles...nothing remained. I don't think she gave a shit.
Have you ever noticed this?

She suffered from health problems in her later years and probably couldn't give a shit about spending two hours every morning doing hair and make up.

I agree with R5. I think she just wasn't VAIN. Even when she was young and very cute she had no self-consciousness about her.

Patty is/was a national treasure.
She also was sick in her latter years, which takes a toll on your body. She had been working since she was about six years old, suffered emotional abuse, became bipolar and came out the other end to talk about it. Eventually, it all catches up with your body and shows on your face. But she was an outstanding actress and won an Oscar and several Emmys along the way. She was very attractive as a young woman, and she had a joy for life and inner beauty as a older woman.

I have met hundreds of celebs and stars and to this day she was the most lovely. Warm and funny she was an itty bitty thing but when she hugged you, you were hugged. In 2002 she was in her barn in Idaho and a horse got spooked and threw her into a post. She got a concussion and a skull fracture. That was in September. In December she was on Broadway in "Oklahoma" as Aunt Eller.

I always thought she was plain in her teenage days and in "Valley of the Dolls." She had a wide face that was by no means conventionally pretty. She had horrible hairdos and wigs that did not suit her throughout the Sixties either.
She looked better when she was a little older--the Seventies fashions suited her better, and though she still did not look conventionally pretty, her face looked soft and pleasant.

Well , that's obviously not true. She's quite obviously in that photo wearing mascara and pancake and lipstick/ lipgloss.
However, I will grant you that in this photo they are absolutely beautifully applied, and she looks the best I've ever seen her look by far.

Patty/Anna went through hell and back, and people still loved her. For many years, half the country grew up with Patty Duke. Affection and love like that for an average girl from New York with big huge talent never fades from people's consciousness. She may not still be your favorite star, but you always remember her fondly.
