Evidently, Lesotho uses the term First Lady to refer to wife of the Prime Minister, separate from the Queen Consort and Queen Mother. Before the first wife was murdered she had gone to court and they confirmed her as First Lady, even though she and her husband were separated, because the divorce was not finalized. That would mean that the country recognizes that title refers to the wife of the PM.
