Stars Promoted To The "And / As Credit."
Those who DID NOT ORIGINALLY have the credit
Also, I don't count those in last position anyway but later on got the "and /as," such as Nancy McKeon
Marian Mercer "It's A Living"
Mary Kate Ashley Olsen "Full House"
Valerie Bertinelli "One Day At A Time" (demoting Pat Harrington in the process)

On DALLAS: Linda Gray, Ken Kercheval, Steve Kanaly, Susan Howard, Priscilla Beaulieu Presley, Sheree J. Wilson all started out with just their names being shown as the episode was starting after the opening credits/theme music.

Howard Hesseman played Bonnie Franklins/Boyd Gaines’ father in One Day at a Time.
The credits on that show changed constantly as they hired/fired/promoted/demoted actors.
The credits change multiple times in 80-81, the season Mackenzie left- First it’s Ann/Schneider/Julie/Barbara. Then Max is added. Then Julie is removed. then Max is removed. Then Gay Alex and his dad Ron Rifkin are added.

R30/r31 it’s always funny seeing very late season ODAAG credits. Unlike some other shows from the second half of the 70s like say Three’s Company, or Different Strokes with that jazzy new opening, One Day at a Time didn’t even remotely try to adapt to the 80s. So you see these credits from 1984 but it still looks like 1977 to me.

Season 12 of E.R. - all of the original cast members had left the show. Instead of bumping Laura Innes to the top/first position in the opening credits, they gave her the senior “And” position. They also greatly reduced her character’s storylines, but the actress had moved to directing more episodes so she apparently didn’t mind.
The top two positions in the credits were given to the boring Croatian and Maura Tierney, who were the romantic leads on the show in terms of storylines and screen time. Sherry Stringfield was supposed to be on that season but unexpectedly did not sign a new contract over the summer (she appeared in the first episode of the season only because they had filmed it before summer hiatus). If she had stayed she and Laura would have had to duke it out for the “And” position.

Similar to “It’s A Living”, St Elsewhere had two “And” positions in the title credits - one for Norman Lloyd and one for William Daniels. When star Ed Flanders left the show, they promoted Daniels to the top star position. So I guess the second “And” position had more clout...take THAT Ann Jillian!
(Also, why didn’t Terence Knox have a bigger career? He was absolutely gorgeous and has aged very well.)

One of those actors should have gotten the "with" that comes right before the "and". There should be only one "and"
The only acceptable titles are:
"Starring"
'Co-Starring"
"And"
"With"
"Special Guest Star" or "Special Guest Appearance By" or "Special Appearance By"
and
"And Introducing"

Stargate SG-1: Micheal Shanks was credited as "And Michael Shanks as Dr. Daniel Jackson" from season 7 onward.
The reason for this was that Shanks left the show at the end of season 5 after longstanding disagreements with the show's producers. During season 6, it became apparent that the show wasn't working without his character and they began to try and get him back. Meanwhile Shanks thought he'd make it big in Hollywood after leaving the show, but later recounted that he actually only got one audition the whole time. So he agreed to return. He was given the 'and/as' as kind of a 'welcome back' gift.

That is so true. I saw an interview with him and he went on and on about how "cute" she was and they wanted to make her a tomboy but she was just too cute for that, so they changed it right away. Then he went on about how good she was and he said, that MacKenzie was a star but Valerie could hold her own with her. He even said, how brave she was to sing with Mac, (being Elton John to Mac's Kiki Dee) as Mac was from a famous singing family.
