I think the trouble with conceiving only happens if you get so thin your periods stop, which no doubt affects ovulation, etc. However, I think most of the Windsor babies are conceived via IVF to ensure the right gender and right timing of the pregnancy, so as long as Kate doesn't go down to truly anorexic levels, getting up the duff was no trouble.

In Kitty Kelley's dishy royal bio, she said that both Elizabeth and Margaret were conceived via artifical insemination, which was a groundbreaking procedure at the time, because their father, the then-Duke of York, had issues with impotency.
