River died from a drug overdose right outside the Viper Club owned by Johnny Depp. Depp's own long alcohol/drug overdose is catching up with him slowly but surely.

That disgusting junkie John Frusciante of the Red Hot Chili Peppers caused the overdose, he told River to drink something and promised him it would make him feel fabulous, it turned out it had some kind of heroin in it and wasn't to be consumed orally. The Phoenix family were hippies and so covered up John's role. Interestingly enough, in this 1994 clip a drugged out John is blasé about River's death which suggests he didn't give a fuck about him in the first place. Go to the 20 minute mark:
