These types get turned down by reputable surgeons (he was on Botched, the show on E!) - and they turned him down and said do NOT have another nose job or it could rot off because there is a lot of damage to blood vessels.

Well, what did he do? Found another surgeon (in Dubai), got another nose job, and it started rotting off. So, yes that surgeon should be ashamed but at least 90% of American surgeons told him to stop.

While it may be the case for some, I doubt that this is always about dealing with problems or trauma. For so many people (even if the roots may be tied up in not believing that they measure up physically or being told so), this behavior has an obsessive-compulsive component where once they start, they find it challenging to stop.
